Social prescriber link workers at your surgery
Many things affect our health and wellbeing, such as where we live, how we spend our time, our financial situation and our relationships with friends and family.
Sometimes we need some extra support. For example; when coping with a short or long term health situation, feeling lonely and isolated, or worrying about financial and housing issues.
That’s where social prescribers can help!
Social prescribers
It starts with a conversation. It might be the conversation you’ve just had with your doctor or with another person in the practice team. They will refer you to the social prescribing link worker within your surgery.
The link worker is there to listen to you and put you in touch with the people and activities that might help you to feel better and improve your overall wellbeing.
Social prescribing can help you to have more control over your own health and find ways to improve how you feel in a way that suits you.
