We are increasingly seeing patients who have been asked to have a blood test by the hospital or their consultant, arrive at the practice for their blood test but without the required paperwork.
If you have been asked by the hospital or your consultant to have blood tests, please ensure you bring your blood request form with you.
Without your blood request form, we will not be able to carry out your blood test and you will be asked to rebook your appointment.
