Self referrals

There are a number of services locally that you are able to make self referrals to.

GP referrals

To request a GP referral:

Provide as much information as you can as this will help us to process your request.

Tracking a referral

To track an existing referral:

Once a referral has been completed by the practice, it is sent on to the organisation to which you have been referred. This means that we may not have any more information to offer until the receiving the clinical letter from the department after you have been seen. To check for any updates on your referral if you’ve not yet been seen, it is best to contact the referred to organisation.

NHS e-Referral

If your doctor has referred you to a specialist for further care you will be asked to book your appointment using the ‘NHS e-Referral’ system.

You will receive confirmation from us within 2 weeks that the referral has been made, and any passwords you may need.
